I have lost my id card and my name in id card is wrong

Dear sir,

i had lost my id card and name and date of birth in my id card is wrong.as there was my name is kuntal biral in place of kishor kunal, my father name is bacchan bitthal but on id card it is bacchan biral and my date of birth is also wrong.my date of birth is 15-08-1988.So sir please renewal my new voter id card with correct name and D.O.B.my voter id card no is BVH6116321.Now at this time i m in delhi and this voter I.D. card is of bihar.sir plz response on my complain soon.otherwise i will be in problem.my mob no is-9313264698, 9350858294
