Non reciving of passport after expiry of four years

I have applied for Passport against In the Passport office of Bhubneswar against File No. A011765 dated 05/10. Inquiry by police has been conducted and faverable report has been given, but after several aproches and letters and the expiry of four years Passpor has not been Issued to me.
