1. Rs.1000/- DD.No.650470, dated 31-10, State Bank of India, Parvathipuram branch, to words passport fees, Favour of Passport officer, visakhapatnam.
2. M.Com marks list and original certificates.
3. My husband passport zerox copy.
4. PANCARD zerox copy.
5. Cell phone bill.
6. Bank passbook first page.
7. LIC premium receipt.
I am not received any information to passport office. I have not been enquired by any police officer eleven months. I request you to help me out to get enquiry and received my passport.
