Passport missed

Dear Sir,

My passport issued in the year approximately 1999 on the Name of Ramamurthy kasula from Hyderabad passport office

Addess on the passport is Ramamurthy K., C/o. srinivasulu. K, Kovure post, H.No-2-6-45/10, Nellore dist.

My educartional qualifications M.tech Chemical Engineering IIT Delhi.

I missed this passport few months back and I don't remember the passport No. and this is the reason, I am unable to compaint for police equiry.

Can u pls suggest me how to go ahead for locking the old passport and getting the new passport.

At present I am working for well known pharma company in Hyderabad.
