EPF withdrawal amount not yet received

Dear Sir,

I have submitted my PF withdrawal forms to my previous company PLACER PHARMA, Subhash Road, A Vile Parle (East) Mumbai and they have sent it to the EPF, Bandra office, and you sent me letter dtd 15/2 Sub: Settlement of Provident fund A/c. My A/c No. is MH/40042/762 stated the claim is not attested by an authorised official with his official seal.

I resend my document to the company on dtd 30/7 & request them to do the needful but till now, I did not received any reply.

Please look into the matter and get me my Provident Fund as early as possible as my mother & father both are hostpitalised.
