PASSPORT NOT RECEIVED

I applied for passport with Chennai passport office on 20/04.

My police verification was done twice at my old address and once again for my new address.

I checked with the commissioner office and confirmed that my report was dispatched in the 2nd week of January. Again As asked for, I submitted an Affidavit to Chennai passport office in March.

I have not yet received my passport.

please advise
