Passport not received

We apply for a passport of my sister on 25.11.2009 with file ref no: DLH-A061206-09, Applicant's Name: REETA CHAUHAN. I am enquried from passport office he says your all formalities are completed, but status of passport still showing

" Your Personal Particulars (PP) forms are incomplete; pls. forward 3 PP forms with photos for each address (es) where you stayed during last one year."

For last 5 to 6 months every time when i enquried about my passpot he says your passport definitely dispatch next week. But still i am not received my passport.
