EPF form rejected but reason of rejection is unknown

Dear Sir,

I’m Anand Ballabh Tiwari, I have checked my PF withdrawal status in EPFO site and It looks my ASR form rejected due to some reason/rejection letter has been dispatched, but I have not received it yet and I’m not able to send them other form because I don't know the actual reason why PF office reject my ASR form, while investigating the same they suggest me to get register your query in EPFO grievance portal and you got positive sign and I did the same but yet I’m not able to know the status too-

Can you please share me the reason in detail (why and where is my rejected form)-
