MTS offered me "Rebook Shoo" on purchase of data card at 3000/--

Around 4 months I purchased MTS datacard (my MDN number is: 9145042412) under this offer scheme from MTS executive. and he told me rebook shoo will be delivered me with 15 days.

but till now I did not received yet.
