ESIC card accno not given since last one yea

Hi

I am employed with Aurum Soft Systems Limited .15, Besant Avenue, Adyar, Chennai - 600 020

They are deducting regular amount as ESIC contribution from my salary since last one year but not have provided the ESIC card or account number.

When I enquired about it they said come and fill the form.

I filled the form in March but till date they have not processed it. When asked about status they say it is in process.

Please help me in this regard as what I should do.
