I, Subrato Datta, son of Sri Mani Lal Datta on behalf of my father representing the following case for early solution. Copied below is my fathers' representation to on 2-2. [email protected] with a copy to [email protected].
It is most unfortunate that till today no one bothered to acknowledge the receipt of the mail.
You are, therefore requested to interfere in the case and request you to take necessary action for refund of the amount mentioned below in my fathers email.

Hello,
I received the above mentioned policy on 31.1.2012. After going through the policy documents I found to my utter surprise that not only the policy fulfilled what was told to me but also the application form was tampered totally.
The policy was in the name of my wife Bani Datta and Mani Lal Datta. Nominee was my son Subrato Datta. In the Policy docs received I found that my wife's name was replaced with the name of Subrato Datta and keeping the nominee column blank. My wife's name is totally wiped out of the policy.
I found later elsewhere that the maximum entry level age is 65 years. Whereas my wife's age is 70 years and mine is 84 years. Hence I presume that the person dealt with my policy application fradulently replace my wife's name with my sons name who is 45 years of age.
Again at the time of application the person dealing took the pan card xerox of my son, the nominee. Reason not known? Also he was asked for signature in the nominee section. Nowhere I found than except the name and relation any other detail is taken of nominee.
The xerox of the application form as attached with the policy is totally filled with wrong information. The Original application form was filled by my son himself but the enclosed copy is not the same.
While applying I was told that in the Plan Vision, Life coverage is 7 times and Medical Coverage is 5 times the premium. gain at the end of 5 years 10% compund interest, 2% Bonus and 5% Loyalty Bonus will be paid. But such information is not mentioned in the policy.
Hence, I am not interested with the policy to continue. So it is requested to cancel the policy with immediate effect and refund the entire money i.e., Rs 11, 400/- as paid by me as premium.
I expect the matter will be resolve within 7 working days or else the case will be referred to IRDA, Hyderebad
I also expect stern disciplinary action against the person involved with this kind of fraud. For reference I provide detail of the following who dealt with me with the application.

(This person visited my home to collect application form, cheque etc)
3. Verification from Audit Dept. - Rakesh Rai
The first two represented themselves as employees of Birla Sun Life Insurance whereas in the policy document the Advisor name is Snowtex Investment Ltd.
Hence in line with the note of Chief Operating Officer, in the policy, the policy can be cancelled with 15 days and this may be done accordingly.
