Poor respponse in service to custome

I ve blocked my old atm card since it was not working and obtained a new atm card as per bank procedure. the new atm card is not activated by bank even after 15 days and moreover i'm not getting a responsible answer from the branch manager of canara bank teppakulam branch, trichy, mr. nagendra.

i had gone twice to bank personally but still no use. i'm a tax payer.

i need to know the procedure for filing a complaint against these irresponsible people / government servants.
