Efuse to provide form

Dear Concern,

Today i visited your PNB lado sarai branch at 3:00 pm where i asked for an current account opening form, Mr. Solanki who was sitting at counter no-1 refuse to give us form he says that form given and submitted before 2:00 pm as he don't have time to explain us about the procedure for same. When i am asking again still he denied. When i am asking is there any written information for same then he replied this is our branch rule.

Can you confirm me is any time specific for provide us the for account opening form if not pls tell your employee to rectify his behavior and treat customer nicely hence this is an new branch and he must know value of an customer in new branch.

waiting for an favorable reply soon and ask him to improve his behavior.
