Non Receipt of Right Issue Form

I am holding 700 shares each in the name of my father, mother and myself respectively. The right issue has already opened for subscription from 1st of March and till today i.e. 4th March no right issue form has been received by me. Kindly advise how can i apply to the issue and whom to approach in regard to same.
