1. that, I, Malkeet Singh R/o, WZ-116, Plot No.-332, Chand Nagar, New Delhi-110018 have enrolled to the policy (SBI - Subh Nivesh) vide Policy No. 35001761809 having proposal no. 35715422 Dated 31st January was executed through Broker BONANZA ASSURANCE ADVISORS, HEAD OFFICE (Channel Code : 18353026) in favor of SBI Life Insurance and amount was paid of Rs.18000/- (Rupees Eighteen Thousand Only) via Cheque No. 060368 dated 31st January (of Syndicate Bank, New Delhi) having Receipt No. NOA/IP/14524 (Copy attached). <! --smile:--><img style="vertical-align: middle;border: none;" alt="" src="engine/data/emoticons/.gif" /><! --/smile--> AND<! --smile:--><img style="vertical-align: middle;border: none;" alt="" src="engine/data/emoticons/.gif" /><! --/smile--> Policy No. 14018328802 (SBI Life Money Back Plan Option 2) was issued in the name of BHUPINDER SINGH R/o, WZ-2/7A Vishnu Park, New Delhi-110018 having proposal no. 146114092 Dated 10th February was executed through Broker BONANZA ASSURANCE ADVISORS, HEAD OFFICE (Channel Code : 18353026) in favor of SBI Life Insurance and amount was paid of Rs.18000/- (Rupees Eighteen Thousand Only) via Cheque No. 906108 dated 10th February (of ICICI Banking Corporation Ltd, Punjabi Bagh)
2. that, I have enrolled to the above said policy along with Mr. Bhupinder Singh (Another Policy Holder). Please note that, both of us were enrolled to the policy, which was committed and agreed by Mr. Niraj Kumar (the policy seller) that, our policy was issued wrongly by the mistakes of his officials and the policy was UNIT PLUS i.e. only paid for 3 years and get money back after five year. And both of us have cheated by the policy seller.
3. that, Please refer to the Annexure-I for detailed matter and correspondences I have done with SBI Life Insurance as well as with IRDA but nobody is adhering.
4. that, IRDA has asked me to fill the form provided by them to me as their mail-attachment which was filled and mail twice (see: forwarded email) and all details and evidence were provided to both of them, but have no output to come up with till date.
5. that, earlier SBI Life Insurance has agreed to help us by writing reply as

Dear Sir, Please find the attached Form duly filled along with detailed matter as an attachment with proof (Mobile Voice Clips) to support to my claim. Also, please note that, I was very busy due to my marriage held on 19th February resulting delay in response. I apologize for the delay in reply. Further, to furnish you that I have received this kind of reply from you second time stating requirement of filling the form, which I have already done twice. Also, I have talked to the Customer Care of IRDA for complaint. But your representative suggested me to first asked to the Insurance company.
