ICICI Bank Credit Card
Issued card and done insurance without my consent

I got issued a icici credit card for free for one year, after the expiry they issued other card saying the card is free for life. I never used the card, last week i applied for a car loan in axis bank they rejected my loan saying that i have an outstanding amount of 52000 on my icici credit card, i inquired and got to know that they have done a medical insurance on my card without my consent.
