ICICI PRULIFE INSURANCE HEALTH SEVER
Polcey document not received after 4 months

Dear Sir,

I am buying a insurance policy from ICICIPRU LIFE product name Health Sever, but till date i am not receiving our policy document.

I am giving a cheque in month of febuary Rs. 15000.00 in favour of iciciprulife insurance co. to your marketing executive Mr. Chandravadan Mobile no.09974703238, after talking to your office executive Mr. Ketul Shah. But after 4 month now when i want to talk with your executive they will not pick the phone nor he give any detail when i find my policy document or my policy no.

kindly do the needful and send my policy document to my address which is given to you.
