About pan card verification

Respected sir,

My pan no is AFEPN2849H. Recently I was trying to file income tax refund online where I was asked to verify my pan card. When I did so my pan cad was found already registered, although it is my first time to fill income tax return online.

My query is, how did my pan card got registered without my knowledge?

If it is already registered by IT dept, what is the default password to login?

If not, how should I login, and how should I e-file tax returns?
