I Mr. SHREEKANTH D M (PAN NO.CDXPS7275K) has applied for a PAN card few months back. I got a msg from concerened department that your card has been dispatched with above mentioned PAN number. But till today i didn't get my PAN card. So i kindly request you to send my card as soon as possible to following address.
