Non receipt of LIC Health Card

Sir,

I would like to bring in your kind notice that I have a policy with LIC of India No. 124021872 (LIC' Health Plus) and Rs.10, 000 is being paid everyear as a premium since 2008. I am very much regret to note the poor service rendered by LIC of India as I am still waiting to get the TPA Card of Myself and for my son.

Now, I would request you to intervene in this matter and resolve the issue.
