{My brother in law Late Ramen Chandra Das had his last breadth on 22.06. 01.30 AM at Down Town Hospital, Guwahati, Dist: Kamrup, Assam due to primary cause of Ca-Oesophagus-I (Cancer in Oesophagus which was detected in 3rd Stage: The exact cause of death). Approximately four month ago he had signed in the policy and paid the initial instalment Rs. 125000.00 as he was in Indian Railway Service; posting at Silsar. After death of my brother in law, my widowed sister while contacted LICI office of Silsar (where the deceased had made the policy No. 491946634) provided information as per the standard form 3816. After receiving the information as per the form no. 3816, LIC Silsar office again requested for additional information as per the form no. 5152 and documents as-

1. Bed Head Ticket/Case History/ Sheet/Discharge summery and biopsy report

2. Enclosure Claim form 5152 from Dr. U Islam

3. Treatment particulars of Ca-Oesophagus along with biopsy report.

After information provided in the form 5152 (date of the enquiry letter from LIC, Silsar was 22/2) Dr. Abidur Rahman transferred left down town Hospital and Authority of Down Town Hospital expressed their inability to provide the Bed Head Ticket as they informed my sister that they don’t store so old record).

At last LIC Silsar office set deadline of three month which was over by July’ 2010 to provide the Bed Head Ticket, otherwise they refused to further process the death claim and my sister unable to provide Bed head Ticket (As hospital Authority did not co-operate).

LICI making these queries as the case is an early claim?

1. Now my question is are the bound to provide such types of information to LIC? Why LIC can’t made enquiry themselves?

2. In this case the last treated hospital was the hometown (Guwahati, Assam) of my Brother in law/sister, what would have happened if the death would have occurred at Apolo, Chennai or Tata Memorial Hospital Mumbai? Are a widow (the claimer) bound to gives such documents/information?

3. Is LIC can terminate a claim process like this?

4. How my sister can have the right judgement? Or how/ to whom she has to approach?
