PAN CARD not received

Dear SIr/Madam,

I, P.Sumitha, IRS am an Assistant Commissioner of Income Tax (UT) from the National Academy of Direct Taxes, Nagpur. I would like to submit that I had applied for a PAn atleast 4 months ago n the tracking PAN status displays the message that my photo could not be scanned properly. So I would like to know the next course of action that I am expected to take for getting my PAN as early as possible.
