Provident fund claim not yet settled

I, Raj Kumar Saraf, having PF No: MH/40890/163, has resigned from my ex company, and my ex-employer has submitted Form No. 19 & 10-C filled and duly endorsed, for withdrawl of full amount. Although these forms have been received by deptt. on 24th August, and acknowledged by them, but even after 5 months of the same, my PF amount accumulation with interest has not been transferred to my nominated bank account. Please look into this case and arrange for immediate transfer of dues, as I need the same most urgently.
