Pan Card not recieved

I have a pan card still, but it happened some mistakes, my date of birth and my name spells mistakens found. I proposed and corrected. I acknowledge also corrected from internet. But that corrected PAN Card not recieved still now. Please kindly send pan card to the following address asap.
