PAN NO RECEIVED BUT CARD NOT RECEIVED

I am working in Indian Air Force, and I have applied for the PAN Card through Service sources, and I got my PAN No. in my PaySlip since long years Back. Since i didn't receive my PAN Card. i have approced through various agencies but i got a response all over that my address or name doesn't match So please advise how i can get my personal details as same in the PAN Card. without having a PAN Card i am facing lot of problems like taking a credit card..etc.
