With reference to the above I hope you might have received my letter dated 5th November along with withdrawal Claim Form No.10C & Form 19, which is duly filled and signed by me. As I do not have any direct contact with the Employer, so far, as per your instruction I have attested my application through Manager, State Bank of India, City Branch (Code 0814), Bangalore-02, where my SB account is laying.

I request you to kindly process my application settle my claims as per my contributions made. In case if you need further any clarification you may communicate to me.
