Its been three years and haven't recieved my Provident fund

Dear Sirs

I left the above company 3 years ago and filled the form for the provident fund 3 times, but I haven't recieved my provident fund yet. Whenever I ask the company they suggest me to fill another form as they lost my previous forms.

I have been asking for my PF reference number from the company so that I can enquire in the PF office or rather make a complaint in the PF Office, but it seems company is not at all co-operating to help me in this case.

Kindly look into this and suggest me if there is a way to get my provident fund.
