Not given proper reference check

Hi Team,

I have one seroud doubt about the norms / policies of the company. once they have issued proper relieving / service certificate with clean chit is there any chance to give the feed back as "terminated from the services" although the employee resigned from the company himself. Could you please help me to go further level of process
