Can any one help me to draft a request mail asking my 3 months pending salary and other dues from x company

Hi, I joined retail company and i have been ask to leave from that company after successfully worked for 17 months with giving any prior notice

At the time of leaving, Vice President assured me ill get my all dues including salary, But that company

Now they have not yet released my salary & other dues (Pf)

I have been trying to get through HR department but hr is not responding from past no reply from his end from last Oct-10.

So can any one suggest me how to write a polite mail to my VP by requeting her to release my dues. can any one help me please?
