DEAR SIR,

Kindly refer your letter ref. no. DL / 32815 / 35 / 5830 DT. 9.11.2010 and DL / 32815 / 35 / 6095 DT. 5.2.2011 in response of my request submission of my PF contribution deducted by my previous employer M/S G.K.DAIRY & MILK PRODUCTS PVT. LTD., NEW DELHI .

As per your office requirements, I have submitted all the related papers with the relevant form no. 19 and 10 C duly verified by my banker ICICI BANK LTD., when my employer not verified my form at that time.

But Still, I have not received the withdrawl of my contribution till date in my bank account no. 630001500890 ICICI BANK LTD., DEFENCE COLONY, NEW DELHI .

I came to know by your office letter DL / 32815 / 35 / 6095 DT. 5.2.2011 that verified form 19 and 10 C have been sent to my previous employer and the 3 months have been passed and my form is still with my previous employer' office.

Kindly do instruct to the concerned to act legally in this favour and instruct to release my money in my bank account no. 630001500890 in ICICI BANK LTD., NEW DELHI
