TO get duplicate consumer card due to lost the original

Sir,

We have lost our Gas connection card and we don't have regarding papers also.

It was on the name of my father Late Sri Bharat Bhushan Barchhiha. I asked to my dealer for duplicate connection but he is asking for papers.
