Non issue of pass book registered more than 8 and half months lapse

I have a new gas connection in my name registration on 19/5 vide No. 111607970002459 runing sl. no. 10817 to our nearest distributor NAHAR GAS AGENCY, COLLEGE ROAD, HAILAKANDI, But till date I have not found gas pass book inspite of repeated requst for which I am find trouble to refilling cylinder.
