Gentlemen,

Subject: Payment of Safety Bonus – Regarding.

I am a member of Road Safety Club programme launched by Road Safety Club Pvt. Ltd; (photo copies enclosed) and I am holding the Silver Card vide Certificate No. 102839 dated 30.09.2002.

As per the terms and conditions the period of validity of the said membership is eight years from the date of issue and if no claims are made during the tenure of membership an amount equivalent to the membership fee would be paid as safety bonus after the membership expires.

My membership has expired on 29.09.2010 and I have not claimed any benefit during the membership period of eight years.

I have many times contacted to your Bodhan Office as well as in your Hyderabad Office but they answered that till today they have not received any safety bonus cheque from your office.

Therefore it is requested to send the safety bonus equivalent to my membership fees of Rs. 4, 000/- [Four Thousand only] which is paid vide your receipt No. 0259225 dated 30.09.2002 [photo copy enclosed ]. Please do the needful at the earliest.
