Not recording our request for refill cylinde

Our consumer no. is 1152. We have 2 cylinders & presently both the cylinders are empty (as on 05.03.2010). Since first week of February we are requesting to book refill LPG cylinder with our service provider GIRISH GAS SERVICE, Dharavi, Mumbai 400017. Various excuses are being given for not delivering the cylinder e.g. House is locked, refill has been supplied four days ago now wait for 15 days. Booking no. alloted to us was 78 on 08.02.10. Booking no. was changed to 848 on 04.03.10 when my wife contacted. In between these dates I personally visited shop and was assured that cylinder will be delivered in 4 days. Today is 05.03.2010 & still both the cylinders are empty.
