Non issuance of account slip of Employees of code MP/19725

Regional Pf Office indore has given coverage to our institute from 1996 and forfeitted an amount of Rs. 49, 67, 745/- from our bank on 16.09.2010 by taking action under section 8 f. We have already submitted 3A, 6A, form no. 9, specimen card, from no. 2 and 5 of all employees. but still our institute had not got annual account statement of employeee. Inspite of several reminders and meeting with pf official our employee have not got annual statement. we are regulary depositing amount in code MP/19725. In mean while our one employee was expired and his family is in trouble but due to non receipt of annual statement and non cooperation of pf official his case has not been solved yet. So on behalf of employees we request you to take necessary action in this regard.
