Online Application Submition problem

I have purchase a entrance exam form with name (Priti Katiyar for M.Ed))

from HDFC Bank yesterday (03/05) today when i am login on your website

then it is showing that form allready submitted in this login name. The

person name is Mr. Arvind Kumar Verma (Mob-9453912798) which was submited

form with this Login.

When i asked from this person login and pass that is different but same form

submited on both login. Please do needfull as soon as possible.
