his I.P. NO. IS 1306640298

He had photo captured at I.G.I. HOSPITAL, JHILMIL, DELHI on 27/03

but till date he has not received his SMART CARD.

We have already confirmed with related ESI Local Office NIT FARIDABAD but we were unable to find it.

you are requested to kindly look into the matter seriously as the employee is in big trouble.
