Non receipt of Annuity payments

Dear Sir/Madam,

With reference to the above I the undersigned wish to bring to yur kind notice that I have not received the annuity payments for the above annuity held by me which were due on 1-10 and 1-1.

Your office had sent me a letter dated 27-11 asking for my new account number and the 9-digit MICR number of my bank for reconfirmation. I had promptly replied to the same in my letter dated 23-12 alongwith a Xerox copy of the cheque which was sent via Vichare courier dated 23-12. In spite of this I am surprised to note that the above annuity dues remain unpaid.

I am once again furnishing herewith all relevant particulars for your perusal and record.
