My wife has given birth to a baby boy in Jayshri Maternity Hospital in Feb-12, for medical insurance I needed treating doctors certificate with Obstetric histroy and GPA details, I approached doctor twice but she has given only general certificate mentioning name of patient and date. As insurance compnay require GPA details as well after requesting twice doctor is not giving required details and behaving very rudely. I need that certificate and want the doctor to be penalized for denying consumer right with un-professional attitude.
