New policy given instead of renewal

Sir

I am policy holder under KARVY-NIC Health plan

I joined the scheme along with my wife in Feb with UHID FHKN 0001329625.

This was to expire on 27 Feb.

Before the expiry, on 21 Feb I paid my renewal premium along with renewal request-form.

My premium cheque was encashed on 26 Feb a day before expiry of my first policy.

However when I recieved the new medical cards I found that the renewal status has not been granted to me and the policy has been issued from 25 March onwards with new UHID Number NIC 0008664924.

Kindly take to correct this anamoly

Please note that the same correction may be incorporated with my Spouse K.ATCHAMAMBA (UHID No. FHKN 000 1329626 for 11, NIC 000 8664925 for 12)

Your prompt and quick action will enable to settle my claim with FHP
