Hi Mr Omkar

As discussed, last month with you all the documents were provided to your phagwara office please refer to my appended mail it gives every justification of the confusion you are having with regards to the process.

As per our last telcon you was suppose to call me post discussing the same with your phagwara office and I am highly disappointed with the process follow up done by your company.

This is with reference to the letter which we have received on 18-Feb-11 for below mentioned points:-

Kindly Provide duly filled Oriental Claim form &-gt; The same has been provided to Phagwara office and they have confirmed the same as well

Kindly provide first consultation paper for the alignment &-gt; There is no first consultation paper as when we went to the hospital first time they admitted us

Kindly provide all investigation reports charged for in hospital bill &-gt; The same has been provided to Phagwara office and they have confirmed the receipt as well

Kindly give reason for delay of submittion of documents &-gt; the same was submitted on time to the Phagwara office.

Please confirm the receipt for the same email so that we make ourself sure that our claim is in process.
