I was admitted at the Mission Hospital, Durgapur for Angiography from 11.06.2010 to 12.06.2010.Subsequently I submitted all the relevant documents to the representative of their Agent Mr Manoj Tekriwal on 15.06.2010. For lodging my claim of R/s 12000.00.Thereafter whenever I contacted Mr Tekriwal, I received their assurance. But still as on today my claim has not been settled.

Then I personally met Mr. Atunu Banerjee, of M/s Reliance General Insurance company limited, at Kolkata office and submitted Xerox copy of all relevant documents. Mr Banerjee asked for original copy, but all original documents had all ready submitted to Mr Tekriwal and the same are lying with them.

I shall now request you to kindly look into this matter and take an immediate action to settle my claim and in future nobody will suffer/face this such of problem.

Please note that Mr Tekriwal was appointed by M/s reliance as a agent, not by us. So they cannot avoid the responsibility.
