I have requested the Hospital to provide me following documents for Mediclaim
1) Original tax invoice of stent
2) Duly attested and paginated Xerox copies of the indoor case papers with history sheet and nurshing chart (full set)
3) All original lab test report
4) Original Angioplasty report
The hospital provide me last two items and not provide first two items and now patient co-ordinator is not replying properly
