Request for Claim Sanction

Dear Sir,

Group Mediclaim Policy No. 221000/48/11/41/00000086, Period 01.04.2011 to 31.03. from United India National Insurance Co. Ltd.

Request for sanction of claim for the cataract operation in the name of Mr. Ram Avtar Aggarwal (PHS ID is 11326681) covered under the above mentioned policy.

We have to submit that 2 year back Cataract operation was done for one eye for spouse of our employee named Mr. Ram Avtar Aggarwal and the actual cost was approx Rs. 65, 000/- born by the insurance co. Now the doctor has prescribed the similar treatment for the second eye for the same patten, so our expenditure will almost be same as incurred 2 year back.

Now TPA (Paramount) has refused to give the similar claim due to GIPSA clause. As GIPSA is not mentioned in our policy, hence not applicable to us. It is requested to sanction us the claim on the similar guidelines as given 2 years back for the first eye.
