I have a Savings Bank account with State Bank of India (1853) 22, Taylors Road, Kilpauk, Chennai - 600 010. A/c no. 30745256497. I had been issued an ATM card with no.6220180185300136149.

I request the Bank to immediately suspend all transactions on this card and cancel the same with immediate effect under advice to me.
