I am to inform that I have been operating S.B A/c 010925374733 in State Bank of India, Palasa Branch for the last 5 years. I have opened this account during the period of my posting as A.C.T.O.in the O/o the Commercial Tax Officer, Kasibugga Circle. I am getting my monthly salary through this account till today, though I was transferred during 07/07 from Kasibugga to Vizianagaram. I have been continuing this account, since my personnel loan was attached to this account. I have intimated this earlier. However in recent past I have noticed the following discrepancies in my account as per slips generated from ATM counters:

1. I have tired to withdraw Rs. 5000-00 on 12/06/09 from ATM counter of State Bank of Travancore, Diomond Park area, Visakhapatnam, but in the slip generated “the transaction could not be processed.”. message was displayed. I have faced such type of incidents earlier also. But in no incident amount was not debited from my account. But in this incident, the amount was debited from my account. I have notice this through slip generated on 17/06/09 & the mini statement generated on 11/07/09 also.

2. Further vide ATM slip generated on 04/07/09, it is notice that an amount of Rs. 17687-00 was available in my account. But vide slip generated on 11/07/09 it is noticed that the balance available in my account is Rs.12287-00 only. I could not know how the balance was reduced. I have not operated my account during the period from 04/07/09 to 11/07/09.

I have already informed the 1st issue vide reference 1st cited above. But till today the problem was not rectified. But till today these discrepancies were not rectified. I felt very sorry with these discrepancies and humble request to rectify them at an early date so as to enable me to withdraw my salary amount. I further request to send a copy of my S.B A/c as well as loan A/c attached to my salary account at an early date so as to enable me to check all the previous transactions.

Further I invite the attention to section 6 (1) of the Right to Information Act 2005 where in it is stated that every Public Authority has to respond to any request made by any applicant in time as stated in the act. I quote the provisions of the act briefly as under:

Section (6) (1)

A PERSON, WHO DESIRES TO OBTAIN ANY INFORMATION UNDER THIS ACT, SHALL MAKE A REQUEST IN WRITING OR THROUGH ELECTRONIC MEANS IN ENGLISH OR HINDI OR IN THE OFFICIAL LANGUAGE OF THE AREA IN WHICH THE APPLICATION IS BEING MADE, ACCOMPANYING SUCH FEE AS MAY BE PRESCRIBED, TO—

(A) THE CENTRAL PUBLIC INFORMATION OFFICER OR STATE PUBLIC INFORMATION OFFICER, AS THE CASE MAY BE, OF THE CONCERNED PUBLIC AUTHORITY;

(B) THE CENTRAL ASSISTANT PUBLIC INFORMATION OFFICER OR STATE ASSISTANT PUBLIC INFORMATION OFFICER, AS THE CASE MAY BE,

SPECIFYING THE PARTICULARS OF THE INFORMATION SOUGHT BY HIM OR HER:

PROVIDED THAT WHERE SUCH REQUEST CANNOT BE MADE IN WRITING, THE CENTRAL PUBLIC INFORMATION OFFICER OR STATE PUBLIC INFORMATION OFFICER, AS THE CASE MAY BE, SHALL RENDER ALL REASONABLE ASSISTANCE TO THE PERSON MAKING THE REQUEST ORALLY TO REDUCE THE SAME IN WRITING.

Further as per section 7 (1) of the said act, every public information officer on receipt of a request under above section as expeditiously as possible either provide the information on payment of such fee as may be prescribed or reject the request for any of the reasons specified in section 8 and 9 of the act. I quote the section of the act as under:

7. (1) Subject to the proviso to sub-section (2) of section 5 or the proviso to sub-section (3) of section 6, the Central Public Information Officer or State Public Information Officer, as the case may be, on receipt of a request undersection 6 shall, as expeditiously as possible, and in any case within thirty days of the receipt of the request, either provide the information on payment of such fee as may be prescribed or reject the request for any of the reasons specified in sections 8 and 9:

I further inform that I have already sent this letter two times earlier vide references cited 1st & 2nd cited. Vide ref 2nd cited, I have sent E –Mail to your office. But no reply was received till today for which act I hereby express my deep regrets. I was suffered economically and psychologically because of these discrepancies.

Therefore I once again humble request to do needful in this matter at an early date for which act of kindness I shall be ever grateful to you..
