Unavailability of internet password

Dear sir,

I Mr. Koushick Datta, a resident of House No. 492-R (Miglani House), Model Town, Panipat-132103. has applied for a broad band internet connection, long time ago. Modem has been delivered at my house a week ago. However, internet password is still not delivered. Kindly, look into the matter.
