Pan card not received

Dear Sir,

I have applied in income tax office for PAN no in year 2003 jan but due to some problems only PAN No is alloted to me and PAN card not received yet, so kindly help me to get my PAN Card. My PAN No is ACZPT9258N. I also want to change my name on my PAN card. PLease help me.
