Loss of bsnl sim with mobile

I have lost my Mobile phone which has the BSNL Prepaid connection @ 10:00a.m dated 9 aug. Request you to kindly deactivate the SIM Card to avoid misuse of my mobile number immediately. Please block my cell no: 9464012550 issued at BSNL, Jalandhar, Punjab which is issued on my Mother's Name i.e Chandan Mala.

Please issue a replacement SIM card. Let me know where to collect it and what are the charges? my alt cont no.9463631520
