This is to bring into your urgent notice that I had lost my ATM cum debit
card of your bank a long ago and had brought this matter into the notice of
competent authorities.
At last I have taken this platform to put up my grievances with the hope
that my complaint will soon be addressed giving some relief to me. Needless
to mention the financial losses and mental trauma through which I have
passed.
Kindly request you to take a prompt action in this regard and
issue me a duplicate card.
